NEW Empire BCBS ID Cards / Optum - (1/16/2020)
As you may know, Empire BCBS recently mailed new ID Cards, making your old ID Card invalid. You will need to present the new ID Card when seeking medical/hospital services.
Below is a summary of changes reflected on your new ID Card.

EAP/Behavioral Health Services: Effective 1/1/2020 these services will be provided by Optum. Their Member Services Phone Number can be found on the back of the Empire BCBS ID Card. 1-844-884-1852
