Local 1 Certification Requirements - (10/6/2020)

Weekly Unemployment Benefits and Extension of Eligibility Claim Forms Require Certification Section "D" to be completed by Local 1:

If you are unemployed, ready, willing and able to work. Below is a list of all the documents, necessary for Local 1 Section "D" Certification. These documents must be submitted to Local 1 in person at the Union Office:

          • VALID PHOTO IDENTIFICATION 
          • VALID U.A. LOCAL 1 UNION CARD 
          • MEMBER IS ON THE OUT OF WORK LIST    
          • MEMBER IS IN GOOD STANDING                              
          • PROOF OF STATE UNEMPLOYMENT BENEFITS
          • 2 YEARS WORTH OF W2s OR 6 MONTHS OF PAY STUBS
          • COPIES OF CERTIFICATIONS (OSHA 30, SST, F60/G60, ETC.) 
          • COPY OF NYC DOB REG. JOURNEYMAN CARD 
          • NAME OF LAST CONTRACTOR AND JOB LOCATION

Once you have Section "D" reviewed and certified by Local 1, you simply submit the completed  Weekly Unemployment Benefit Form and / or the Welfare Fund Extension of Eligibility Form to the Fund Office.
