BEFORE YOU COMPLETE AND SUBMIT THE APPLICATION FORM BELOW, YOU MUST READ THIS DOCUMENT AND HAVE THE FOLLOWING ITEMS IN HAND.
- Have your OSHA 30 Card (less than 5 years old) or Training Completion Certificate (less than 90 days old).
- Have your 8-hr Fall Prevention Training Completion Certificate.
- Have your 2-hr Drug and Alcohol Awareness Training Completion Certificate.
After review of your application, you will be notified by email whether you are ready to go to the next step, or if something needs to be fixed.
Warning: We cannot accept illegal or fake OSHA 30 cards for purposes of issuing an SST card. We are required, as an Authorized DOB training provider, to report anyone attempting to use an illegal or fake card to the Supervising Investigator of the Training Compliance Unit at the Office of the Buildings Department Marshal.
